Helping veterans across the Four Corners find their way to care.
A community-based care navigation program connecting veterans across Colorado, New Mexico, Arizona, and Utah to the support, services, and people they need — close to home.
Veterans Crisis Line · 24/7
If you or a veteran you know is in crisis, help is available right now — free, confidential, 24/7.
Why this, why here.
Across the Four Corners, services for veterans exist — but they are fragmented, scattered between agencies, and difficult to navigate. Rural distance and isolation compound the problem: a veteran in a small town may sit hours from the nearest VA clinic, with no clear map of who to call first. The gap is rarely a lack of care. It is a lack of connection.
We don't replace care. We help veterans reach it.
Navigation, not treatment. Five steps, repeated as long as a veteran wants us alongside them.
- 01
Intake & Assessment
Understanding each veteran's needs, priorities, and barriers — listening first, in their own words.
- 02
Care Navigation
Connecting to providers, scheduling appointments, and following through until the door is open.
- 03
Barrier Reduction
Helping with transportation, cost, paperwork, and the system fragmentation that stalls progress.
- 04
Community Connection
Peer support, local gatherings, and a sense of belonging close to home — not just a phone tree.
- 05
Ongoing Follow-Up
Continuous engagement, not one-time contact. We stay with each veteran for as long as we're useful.
We are not a clinic. We are the connective tissue that helps existing care actually reach the veterans who need it.
